       FREQUENTLY ASKED QUESTIONS CONCERNING SCHOOL IMMUNIZATION

For students with medical exemptions from school activities, a physician's note is required yearly.

For students requesting conscience objection of immunization; written request must be submitted to:
Texas Department of Health, Bureau of Immunization and Pharmacy Support, 1100 W. 49th Street, Austin, Texas 78756.  (800) 252-9152.
